How To Add A Custom Form Extension To A Measure

Link a custom form field to a measure so that when grantees fill out the form, activity data is automatically created for that measure.

Prerequisites: To link measures, you must have an existing measure on the award, permissions to edit Measures and Performance Forms, and a performance form with numeric field(s).

1. Click Program Management > Awards.
2. Click the    (View Award) icon next to a grant name.
3. Select Custom > Form Configuration.
4. Under Related Objects (Form Extensions), select checkboxes for which forms you would like to link next to Grant Deliverables.
5. Select checkboxes for which forms you would like to link next to Grant Measures.
6. Click Save.
