How To Add a Custom Form Extension to a Measure

Prerequisite: You must be an Organizational Administrator.

Custom Form Extensions allow you to capture additional data on measure records beyond the standard fields.

1. Open Administration > Custom > Form Extension Library.
2. Click the    (Add icon) in the Icon Bar.
3. Enter the Form Extension Name.
4. In the Applicable Object(s) dropdown, select Grant Measures.

Note: Enable Form Extension on All Records for Object(s) to force it on for funders. Enable Form Extension for Recipients Only to force it on for recipients only.

1. Add a form extension description (optional).
2. Build the form fields and click Publish.
3. Go to Grant Management > All Grants.
4. Navigate to the measure record by clicking on a grant name and going to Custom > Form Configuration.
5. Click on the newly created form under the Grant Measures section.
6. Complete the custom fields in the form extension section.
7. Click Save.
