Custom forms allow funders to collect additional information from recipients beyond the standard grant record. As a recipient, you can submit custom forms directly from the grant record in Euna Grants, and your funder receives the submission for review. If a form is rejected, you can make the requested edits and resubmit directly from the grant record or from the link in the rejection email.

Submitting a Custom Form
To submit a custom form as a recipient:
- Navigate to Grant Management > Grants.
The All Grants page displays. - Click the name of the award that has a custom form.
The Award Details page displays. - Click the Custom tab at the top of the page.
The Custom dropdown menu displays all custom forms related to the grant.
Note: The Custom tab will only display when a custom form is available. - Select the name of the form you want to submit.
The form displays in a grid view displaying all previously submitted instances. - Click the Create button in the options toolbar to create a new instance of the form.
The custom form displays. - Enter the required information and complete the form.
- Click the Submit button.
The form is submitted to your funder and the status in the grid view displays as Submitted.
Note: There are no automated email notifications when a form is approved. If approved, the form remains in Submitted status. If the form is rejected, Euna Grants sends an email to the recipient grant manager with comments outlining the edits required.
Resubmitting a Rejected Form
If your form is rejected, you can access it directly from the link in the rejection email or by navigating back to the grant record.
To resubmit a rejected form:
- Navigate to the forms list, or
- Click the link in the rejection email to open the form directly.
- Click the Edit button next to the instance of the form that requires edits.
- Make all required edits.
- Click the Save button.
The form resubmits to the funder.
