Understanding Application Forms
An application form is a customized form that captures applicant information and responses, allowing applicants to express interest in receiving an award from your opportunity. Every application form includes a title, one or more sections, content-only fields that provide instructions or information, and entry fields that request information or ask a question. An application can include one form or multiple forms.
Choosing One Form or Multiple Forms
Whether you use one form or multiple forms depends on the volume and relationship of your questions. Use a single form when you have relatively few questions, want applicants to see everything on one screen at once, and your questions are closely related. Use multiple forms when you have a large volume of questions with a clear distinction between the areas you are asking about.
Scoring is another factor to consider. To reduce steps for your reviewers, keep all scored questions on one form.
Using Required Questions
As you build your form, you can decide whether a question requires a response. Required questions display with an asterisk for applicants, who must answer them or complete the associated field before submitting. Applicants cannot submit an application while any required field is empty.
Content Field Types
Content fields provide information to applicants without requiring a response, such as instructions or other details. Euna Grants includes the following content field types.
| Field Type | Description |
|---|---|
| Text Label | Provides brief instruction in a few words, such as Enter response below. |
| Description Text | Provides multiple lines or a paragraph of detailed instruction, including numbered lists |
| External Link | Provides applicants a formatted hyperlink to a website |
| Attachment | Lets applicants view or download files relevant to the application |
Question Field Types
Question fields let you request or require a response from the applicant, so you can gather the information, data, or files you need during the application process. Euna Grants includes the following question field types.
| Field Type | Description |
|---|---|
| Single Line Text |
Gathers short text responses, such as an address, name, or phone number. Supports a character limit. Best for basic, applicant-specific content |
| Multiple Line Text |
Gathers long text responses, such as narratives. Supports a character limit and a formatting toolbar for bold text, bullets, and more. Best for detailed, applicant-specific questions |
| Numeric Field |
Gathers numeric responses, such as expected cost, number of residents, averages, or percentages. Can be formatted as decimals, whole numbers, currency, or percentages |
| Multiple Choice |
Best for yes-or-no questions or a list of specific responses, since applicants can select only one response. Standardizes responses and prompts a specific answer |
| Multiple Selection | Functions similarly to Multiple Choice but presents the response options as checkboxes instead of radio buttons, allowing applicants to select multiple options.
|
| Multiple Selection – Checkboxes |
Lets applicants select multiple items from a list using checkboxes. Best for select all that apply questions |
| Dropdown List | Lets applicants select one or multiple responses from a dropdown, depending on setup. Works well for standardized responses where one or more criteria could apply |
| File Upload | Lets applicants upload one file per question, such as a letter of recommendation |
| Date | Provides a calendar entry format for gathering dates, helping ensure applicants enter dates correctly |
| Table |
Lets applicants enter information in table form. You define the number of columns and rows and their labels |
Note: You can restrict any content or question field to reviewers only. This is typically used to provide reviewers with background information or considerations that do not display to applicants.
